Once your appointment is scheduled, we will send you some quick paperwork directly to your phone or email. Once filled out, you will be ready to see the doctor!

Payment Options
We accept both credit card and debit card transactions. Additionally, we have included a secure payment portal to pay your bill online at your convenience. If you have a health insurance plan, call a MyHealth team member at (615) 647-8998 to verify we accept your insurance.
Emergency Treatment
If you have a life-threatening or severe injury, call 911 or go directly to the nearest hospital emergency room.
