Becoming a patient is easy! Simply call our office at (931) 787-1180 to schedule an appointment for one of our signature IV Drips, or setup a consultation to discuss your personal health needs. New Patient Forms are available at our office for pickup or can be filled out at the time of your scheduled appointment. New Patient Forms will be available on the website in the near future!
Payments
MyHealth operates as a concierge-based practice to best optimize your personalized health needs, therefore medical services are private pay only. Health insurance is accepted for any applicable lab work performed, if approved by our lab company. Please call our office if you have any questions at (931) 787-1180.
